On the function of the mammalian renal papilla and the peristalsis of the surrounding pelvis

چکیده
This is an informal personal review of the development over time of my ideas about the concentrating mechanism of the mammalian renal papilla. It had been observed that animals with a need to produce a concentrated urine have a long renal papilla. I saw the function of the long papilla in desert rodents as an elongation of the counter-current concentrating mechanism of the inner medulla. This model led me to overlook contrary evidence. For example, in many experiments, the final urine has a higher osmolality than that of the tissue at the tip of the papilla. In addition, we had observations of the peristalsis of the renal pelvis surrounding the papilla. The urine concentration falls if the peristalsis is stopped. I was wrong; together, these lines of evidence show that the renal papilla is not just an elongation of the inner medulla. We are left without a full explanation of the concentrating mechanism of the mammalian renal papilla. It is hoped that other researchers will tackle this interesting problem.

Results of Pyeloplasty in patients with Ureteropelvic Junction Obstruction in Pediatric Surgery Center in Bandar Abbas
Introduction: Ureteropelvic junction obstruction (UPJO) is associated with inadequate urine drainage from renal pelvis into ureter. UPJO results from incomplete recanalization of proximal ureter during fetal period which finally leads to hydrostatic distention of renal pelvis and calyces. Diagnosis is based on clinical symptoms, ultrasound, and diuretic isotopic renogram. Various surgical metho...
متن کاملAssociation between congenital defects in papillary outgrowth and functional obstruction in Crim1 mutant mice.
Crim1 hypomorphic (Crim1(KST264/KST264)) mice display progressive renal disease characterized by glomerular defects, leaky peritubular vasculature, and progressive interstitial fibrosis. Here we show that 27% of these mice also present with hydronephrosis, suggesting obstructive nephropathy. Dynamic magnetic resonance imaging using Magnevist showed fast development of hypo-intense signal in the...
متن کاملDiuretic effect on the measurement of differential renal function using 99mTc-DMSA in patients with hydronephrosis [Persian]
Introduction: It has been suggested that calculation of differential renal function (DRF) using 99mTc-DMSA may lead to overestimation of the function of an obstructed kidney. The aim of this study was to evaluate the effect of diuretic administration on the determination of DRF using 99mTc-DMSA scintigraphy in patients with dilated pelvis. Methods: Thirty three patients, aged from 2 month...
متن کاملUrinary reflux to the non-functioning previous renal graft
Nuclear medicine imaging is routinely used for evaluation of perfusion and function of the transplanted kidneys. At present, the radiopharmaceutical of choice for this study in our center is Tc 99m-Ec. Complications of the kidney graft are demonstrable in the early and delayed images. One of these complications is vesicoureteral reflux to the graft or to the native kidneys. We present a 27 year...
متن کاملNasopharyngeal Metastasis Originating From Beta-HCG Being Secreting From Transitional Cell Carcinoma of the Renal Pelvis: A Case Report
Introduction: Transitional cell carcinomas (TCC) of the upper urinary tract constitute only 7% of urothelial carcinomas. The majority of upper urinary tract TCC arises from the renal pelvis, followed by the ureter. The majority of the patients are presented with microscopic or macroscopic haematuria, flank pain, or symptoms due to metastasis; however, it can be asymptomatic in 10-15%. Case pre...
